标题: Frailty management in community-living older adults: Past, present and future

The prevalence of frailty in community-living older adults in the Asia-Pacific region is approximately 3.5%-27%. Frailty can have a significant impact on older adults' physical and psychosocial functions and community participation leading to an increased burden of care to their family and society. Regional and international clinical practice guidelines in frailty management have been developed given the extensive research being conducted in well-developed countries in the past few decades.

Besides giving an overview of the topic of frailty, this session will talk about the common risk factors of frailty in the older adult population and standardized outcome measures used in both research and clinical settings. Up-to-date evidence-based frailty management programs will be shared followed by some new research works. The session will conclude with a brief discussion on the potential adaptation of the existing programs for future implementation in different cultural contexts.
简介: 
Dr Tim Xu is an Occupational Therapy assistant professor at the Singapore Institute of Technology and holds a PhD in Health Sciences from the University of Sydney, Australia. He has over 23 years of experience working with older adults and the stroke population in community rehabilitation settings in Singapore and Australia. In the past 10 years, Tim has been working closely with many Singapore government agencies and community organizations as a consultant or advisor in program development and implementation to promote independent living and active ageing in the community. He has conducted numerous professional workshops to train healthcare professionals and community care workers to be program leaders to facilitate community-based programs for older adults with frailty and pre-frailty in Singapore. He received several awards for his innovation in designing and implementing the multicomponent frailty management program – Say No To Frailty in Singapore in 2019 and 2020. Currently, he is running multicenter randomized trials to test the clinical effectiveness of the Say No To Frailty program in Singapore.
